Ensuring all required parties are present for scheduled services
Failure to meet these requirements may result in delays, rescheduling, or additional fees.
Identification Requirements
Valid, government-issued photo identification is required for all adult clients at the time of service. Additional documentation may be required depending on the type of test being performed.
Confidentiality
All client information and test results are handled with strict confidentiality.
Results are released only to authorized individuals or entities
Information is not shared without proper consent unless required by law
Service Limitations
Lucky Labs provides specimen collection and coordination with certified laboratory partners. Laboratory analysis, result interpretation, and final reporting are performed by accredited third-party laboratories.
Turnaround Times
Turnaround times provided are estimates based on laboratory processing and are not guaranteed. Delays may occur due to factors outside of Lucky Labs’ control.
Mobile Services
Mobile services are available within designated service tiers.
Availability may vary based on scheduling and location
Additional fees may apply for after-hours or extended travel
Right to Refuse Service
Lucky Labs reserves the right to refuse or discontinue service if:
Policies are not followed
Proper identification is not provided
Required parties are not present
Behavior compromises safety or the integrity of the testing process

DNA Testing Policies
Purpose
Lucky Labs provides DNA testing services for legal, personal, and informational purposes. All collections are performed in accordance with strict chain-of-custody procedures when required to ensure accurate, reliable, and court-admissible results.
Types of DNA Testing
Legal DNA Testing (Chain of Custody)
Legal DNA tests are performed under documented chain-of-custody procedures and may be used for:
Court cases
Child support
Custody matters
Immigration purposes
Proper identification and witnessed collection are required for all parties.
Non-Legal DNA Testing (Informational Only)
Non-legal testing is intended for personal knowledge only and is not admissible in court. Chain-of-custody procedures are not required for these tests.
Identification Requirements
All adult participants must present a valid, government-issued photo ID at the time of collection.
For minors:
A parent or legal guardian must be present
Documentation (such as a birth certificate) may be required
Failure to provide proper identification may result in refusal of service or conversion to a non-legal test, where applicable.
Appointment Requirements
All parties being tested must be present at the scheduled appointment unless otherwise arranged in advance.
Late arrivals may be rescheduled
Separate appointments may delay processing or incur additional fees
Mobile collections must be scheduled in advance
Collection Process
DNA samples are typically collected using a non-invasive cheek (buccal) swab.
For legal testing:
All collections are witnessed
Chain-of-custody documentation is completed
Photographs and signatures may be required
All participants must follow collector instructions to ensure sample integrity.
Chain of Custody
For legal DNA testing, Lucky Labs follows strict chain-of-custody procedures, including:
Verified identification of all participants
Secure handling and labeling of specimens
Proper documentation and packaging
Shipment to an accredited laboratory
Failure to comply with chain-of-custody procedures may result in invalid or non-admissible results.
Specimen Integrity
Participants must not:
Eat, drink, smoke, or chew gum for at least 30 minutes prior to collection (recommended)
Tamper with or interfere with the collection process
If a sample is compromised, a recollection may be required.
Turnaround Time
Estimated turnaround times vary depending on the type of test and laboratory processing.
Standard results are typically available within 2-7 Business Days
Expedited services may be available in advance upon request

Results & Reporting
All DNA testing is performed by accredited laboratory partners.

Payment & Refund Policy

All sales are final once specimens have been collected
Refunds are not issued for completed tests
If a test cannot be completed due to client error or non-compliance, additional fees may apply
Refusal of Service
Lucky Labs reserves the right to refuse or discontinue service if:
Proper identification is not provided
Required parties are not present
Policies are not followed
Conduct interferes with the integrity of the testing process
Drug Testing / Donor Conduct
Identification Requirements
All donors must present a valid, government-issued photo ID at the time of collection.
Expired or invalid identification will not be accepted
Donors without proper identification may be refused testing
Minors must be accompanied by a parent or legal guardian
Donor Conduct
All donors are required to follow collector instructions and maintain appropriate conduct throughout the testing process.
The following behaviors are prohibited and may result in a refusal to test:
Failure to follow instructions
Disruptive, abusive, or threatening behavior
Attempts to tamper with, substitute, or adulterate a specimen
Possession or use of unauthorized items during the collection process
Collection Procedures
All collections are conducted using strict chain-of-custody procedures in accordance with DOT and non-DOT protocols.
Donors may be required to:
Remove outer garments (e.g., jackets, bags)
Empty pockets and display contents
Wash and dry hands when instructed
Remain in the designated collection area
Collectors will follow standardized procedures to ensure specimen integrity and compliance.
Observed Collections
Observed collections may be required under certain conditions, including:
DOT-mandated situations (e.g., return-to-duty, follow-up testing)
Prior invalid, substituted, or suspicious specimens
Court-ordered or employer-directed requirements
Observed collections will be conducted in accordance with applicable regulations.
Specimen Integrity & Validity
All specimens must meet established validity criteria, including:
Temperature within acceptable range (generally 90–100°F for urine collections)
Adequate specimen volume
No evidence of tampering or adulteration
Specimens that do not meet these criteria may result in recollection, invalid results, or refusal documentation, depending on the circumstances.
Refusal to Test
In accordance with DOT guidelines and standard testing protocols, a refusal to test may include, but is not limited to:
Failure to provide a specimen without a valid medical reason
Failure to follow collection instructions
Leaving the collection site before completion
Tampering or attempting to tamper with a specimen
Failure to cooperate with any part of the testing process
Refusals will be documented and reported to the designated employer representative, agency, or requesting party when applicable.
Results & Reporting
All specimens (except non-legal rapids) are analyzed by certified laboratory partners. DOT testing results are reviewed by a qualified Medical Review Officer (MRO) when required.
Results are released only to authorized individuals or entities in accordance with federal regulations, employer policies, and privacy laws.
Right to Refuse Service
Lucky Labs reserves the right to refuse or discontinue service if donor conduct, identification, or compliance does not meet required standards or compromises the integrity of the testing process.
Purpose
Lucky Labs conducts drug and alcohol testing in accordance with applicable federal regulations, including U.S. Department of Transportation (DOT) guidelines under 49 CFR Part 40, as well as non-DOT and court-ordered testing requirements. All collections are performed to ensure accuracy, integrity, and defensible results.
Appointment & Payment Policies
Scheduling & Confirmation
Appointments can be scheduled online or by phone. To secure your appointment time, clients may be required to confirm their booking. Lucky Labs reserves the right to cancel or reschedule appointments that are not confirmed.
Arrival Time
Clients are expected to arrive on time for their scheduled appointment. A grace period of up to 10–15 minutes may be allowed. Late arrivals may need to be rescheduled depending on availability.
Same-Day Appointments
Same-day appointments are available but limited. Booking early is recommended to secure your preferred time.
Rescheduling & Cancellations
If you need to reschedule or cancel your appointment, please notify us as soon as possible. Repeated cancellations or last-minute changes may require a deposit for future bookings.
No-Show Policy
Clients who fail to arrive for a scheduled appointment without notice may be subject to a rescheduling fee or required deposit before booking again.
Payment Policy
Payment is due at the time of service unless otherwise arranged in advance. We accept multiple forms of payment, including major credit/debit cards and approved financing options.
Deposits
Certain services may require a deposit to secure your appointment. Deposits are applied toward the total cost of the service and may be non-refundable if the appointment is missed or canceled without adequate notice.
Refund Policy
All sales are final once a test has been completed or specimens have been collected. Refunds will not be issued for completed services. If an appointment is canceled in advance and payment has been made, eligibility for a refund will be reviewed on a case-by-case basis.
Additional Terms
All clients must follow Lucky Labs policies and procedures during their appointment. Lucky Labs reserves the right to refuse service if policies are not followed.
